Appointments & Consultations

New patients are always welcome.

Our staff will be pleased to assist you in any way possible and all efforts will be made to accommodate your schedule.

We currently accept Authorized New Jersey Workers Comp, PIP claims, HMO's with valid referral, IME's(prepaid), No Slip and Fall or Malpractice.

Everyone at our office is at your service.

Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy, on your part, will make it possible to give your appointment to another patient.

Emergencies

Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week by calling (856) 751-6464.

Medical Records

Copies of medical records are available at a rate of $10.00 for chart retrieval and $1.00 per copied page.  Our office will complete and submit disability forms at a rate of $5.00 per form.

Billing and Payment Policies

Copayment is due at the time of your visit for participating insurances.  The patient is responsible if the claim is denied.  Surgery bills are submitted to the insurance company before balance billing the patients.

We will make arrangements for monthly payments to patients with a large balance due, usually in the case of surgery.  Patients must adhere to the payment plan or the account will default to a collection / credit bureau.

Patients will be charged for the office visit in the event of a no-show.

 

Financing

Payment is due at the time service is rendered.  We will bill the insurance company for surgery and balance bill the patient for any amount not covered.
